There are two ways:

- either create a diff (i.e. a patch) from the baseline version
  from which your derived version emerged, and your current version,
  then apply this patch to a newer version;

- or, create a diff (i.e. a patch) from the baseline version
  from which your derived version emerged, and a new version
  which is your goal, then apply this patch to your current version;

Depending on the amount of changes you did and the distance
between your baseline and a destination version, one or the other
approach may be more appropriate. In general would think the first
approach is more likely to succeed with the least amount of manual
labour still needed. That could be something like (assuming
file 'amavisd' is your current version in current directory):

  diff -b -U2 amavisd-old-baseline amavisd >my.patch
  mv amavisd amavisd-my-modified-version-backup
  cp amavisd-newversion-baseline amavisd
  patch <my.patch
