I don't know. This is precisely the reason I stopped experimenting
with DSPAM more than a year ago - it just gradually start to become
terribly slow. Perhaps letting it use a SQL database could help.
I'd hope that later development would fix the issue, but I never
bothered to check.

20 minutes, terrible.

Try switching dspam off:
  $dspam = undef;
(or just moving its executable so that amavisd won't find it
on startup), and continue using SA, which seem to be more
predictable/reliable.
