TIMING log entry? amavisd-nanny?
SpamAssassin bayes auto-expiration?

You may also snatch a message that is being processed
for a long time, either from MTA queue or from amavisd
temporary directory, and feed it to a command line spamassassin,
and see if it is slow there too. Make sure to run spamassassin
as the same user id as amavisd is running under.
