The "header: " log entries would show up once for writing a message to a
quarantine, and once for a passed message. If your message _was_ quarantined
but you only see one set of "header: " log entries, and the message
was also passed (spam lovers or *_destiny=D_PASS), then the passed
message really did not receive the X-Spam-* header fields.

Most likely the recipient was not considered local, check the setting
of your @local_domains_maps. Non-local recipients do not get X-Spam-*
header fields inserted.
