I'm not even using the milter (though I'd like to.) and I'm running on 
Postfix blocking 30,000+ messages per day (most through spamhaus),  but 
other's through amavis.  Amavis is painfully slow, but I've found that 
it runs much better on multi-core processors (go figure).  Its also a 
memory hog, but I think part of that is Perl's interpereter. 

In post queue mode, you set the quarantiine/discard number to something 
unbelievably high so that mail doesn't get rejected and then deliver it, 
but mark the header and let the end user decide what they want to do.
