It is an amavisd variable (at least, it lives in amavisd.conf).  It's 
used to add a delimiter to the email address when mail is flagged as 
spam (or a bad header), such as user+s...@example.com, or user+b...@example.com

Useful to get the mail to deliver to subfolders automatically. In 
this case it makes spam auto filter into the Spam folder in each 
user's IMAP folders.

I can turn it off completely, but then I'm losing a major chunk of 
functionality.

I know, and wasn't saying it was a postfix variable (I looked back on 
what I said, not sure how I was misunderstood). local_domains_maps in 
amavisd.conf  is where I put it. And that works just fine, and solves 
my original problem. Email that comes in for the secondary is 
filtered, gets the spam headers, and change of subject line.

It just causes a new problem, because mail going to domainB doesn't 
support recipient_delimiters, and so bounces the mail as 
undeliverable (as user+s...@example.com doesn't exist - go MSFT).

incoming mail I don't know the IP, so I don't know how that would work?

I have several cases:
1: external user sends mail to my server for domainA (from any IP address)
2: spammer sends mail to my server for domainB  using it as the 
secondary MX (from any IP address)
3: external user sends mail to my server for domainB when domainB is 
offline (any IP)
4: (internal) user in domainA sends mail to domainB when domainB is 
offline. (domainAs IP address).

I'm not sure how to narrow down to just the email coming in using the 
mail server using it as the secondary/relay MX server. (or the 
reverse, everything but that).
