Are you using unix sock for clamd?
If so, try a TCP socket (on 127.0.0.1) instead ...

It seems to be a permissions issue.

but everything seems to be correct.
Try adding user amavis to group clamav (as well as adding user
clamav to group amavis, which I guess you already did):
# adduser amavis clamav
