Rick, I have bought my HV fuses from my local Electrical Supply houses. The 
fuses are now made by Cooper or Buss fuse. I am sure you will find the fuse 
part number on their web page.

Where can I find a 1.5 amp, 5000 volt cartridge fuse?  The Littelfuse
catalog number is 70201.5, but it comes up empty on Google.  It measures 5"
X 3/4".

Any help appreciated..
