Yes to all of the above. My cards have a portable/mobile entry field as well. Alternatively, Martha still has AO-7 QSLs that are very inexpensive, and work well for confirming QSOs away from home, or using as reply QSLs for the station you worked to sign and return.
On a related note...if you work a portable/mobile station, be sure to note the grid they were in on the card.
