I suggest you tell us which platform you're on, and what problem you're
having.
If you haven't already done so, you might also like to download
http://www.statslab.cam.ac.uk/~sret1/analog/anlgform.zip
which has a newer version of the form and the documentation. You might find
this a simpler explanation even if you're not on Windows.
