Do you have referrer, query and user-agent info in your logs? Did you define
some search engines in your default config file?

General Summary is always created by Analog. It looks like you've maybe got
elements in the form selecting Filesize and Monthly (or not deselecting them
-- I think they both default to ON).

Change the form interface file. Almost everything that can be done with
Analog can be done from the form. For this and your other questions, look in
the docs (docs/form.html)
