Can you read into DOS box or it's too fast? Maybe there is an error message.

Try to run it from a DOS box using
perl anlgform.pl
or open Event Manager (the place where all errors are stored) and look what
error is generated by the IIS server.
BTW, don't forget you can install Apache on NT ;-)
