Does it work with QV=1? If so, then at least the Perl/CGI part is functioning.
You say the anlgform.pl script works from the prompt but not from the browser?
Have you tried
anlgform.pl > test.html
And looked at the results (you might not be able to do this without pre-setting
some cgi variable in you ENV).
Are you getting anything in your access logs? e.g. 500 errors or 200 ok's?
