Stephen:

Yes, I have followed the troubleshooting guide and I have run anlgform.pl
from the command line with the qv=1 setting.  It appears that it is passing
information to analog.exe but it does not appear that analog is running when
called from anlgform.pl.  Nothing shows up in the server event log.

It is probably something simple but I am not seeing it.  Is there some other
action I could attempt.
