You are writing a little bit less about your problem, but u can be
shure it is an error of your perlscript.
If you have edited anlgform.pl on a Windows-Machine the FileFormat is
DOS, not Unix. Apache and Perl under Linux give "500 Internal Server
Error" in this case.
Convert the pl-script to UNIX-Format should help.

But - as stated above - there is hardly enough information about your
problem.
