By default pages include "*/,*.html,*.htm". The above command should have
produced a warning: you can't put a space in the list. You need a command like
PAGEINCLUDE *.shtml,*.html
although, since *.html is already included, you could just use
PAGEINCLUDE *.shtml
Given your report, you may also want to include *.fm.
