Try this pair of commands:
LOGFORMAT ([%v]%S %j %u [%d/%M/%Y:%h:%n:%j] "%j%w%r%wHTTP%j" %c %b)
LOGFORMAT ([%v] %S %j %u [%d/%M/%Y:%h:%n:%j] "%j%w%r" %c %b)
or this, depending on what you want your virtual host to look like:
LOGFORMAT ([%v/%j]%S %j %u [%d/%M/%Y:%h:%n:%j] "%j%w%r%wHTTP%j" %c %b)
LOGFORMAT ([%v/%j] %S %j %u [%d/%M/%Y:%h:%n:%j] "%j%w%r" %c %b)
