Some possible reasons:
* Someone is using elements off your site, such as images, but not pages,
* Your site has been accessed through proxy "sprayers" where each access,
either for a page or another element, appears to come from a different address,
so your log might have something like
host1.domain.com GET /
host2.domain.com GET /an.image.gif
host3.domain.com GET /another.image.gif
