Yes. Hosts are case insensitive, browsers aren't necessarily, so the
inclusion/exclusion respects that. Local filenames vary according to the
setting of the CASE command.

Yes, you can always specify a case-insensitive match with a regexp. Although
analog won't let you specify a case-sensitive match for a hostname: even in
a regexp it converts it to case-insensitive.
