It's only successful requests.
In fact, it wouldn't be a good guide for failed requests anyway, because the
number doesn't include the bytes in the header, only in the body of the
document. (Well, it's up to your server what to record, but this seems to be
what servers do.) In a short document, including a 404 page, the header is a
substantial overhead.
