If you run with the following settings
REFINCLUDE * # default
REFEXCLUDE *://*
you will get a report only listing requests which had no referrer ("-") or a referrer
not containing "://" (so this will exclude all http, https, ftp referrers).

Keep in mind though that entries without referrer are not necessarily typed directly,
or
referred to from email, some browsers allow blocking or manually changing the referrer,
as do many personal firewalls.
