I thought the top line was the culprit too, I wasn't sure about the
others.

Now the truth comes out -- I was using a modified version of
bigbyrep.cfg sample cfg file.  Once I backed off and used the default
cfg only, sanity returned.  Now I'm gradually adding back the report
parts I need and so far I haven't gotten that bad top line again.  I'll
let you know if I figure out what it was in my cfg that caused the bad
OUTPUT line.
