When I run Analog 6.0 against that log, I get this General Summary:
Successful requests: 23
Logfile lines without status code: 3
Successful requests for pages: 4
Failed requests: 9
Distinct files requested: 23
Distinct hosts served: 1
Corrupt logfile lines: 1

How "empty" is your empty report.html? What does Analog say when it runs?
Have you looked in the error.log file?

Yes (though you may need Analog 6 for IIS5)

Absolutely not - Analog uses those directives to automatically recognize the
LOGFORMAT of the logfiles, so that you don't need to specify a logformat of
your own.
