I just tried the following on my own reports and the
'MySite Website Statistics' appeared as expected as well as the single
line I used as  a test case for the HeaderFile & a single line for the
FooterFile respectively.

Headerfile 'report_top.txt'
footerfile 'report_bot.txt'
HOSTNAME "Some Named Organization"
HOSTURL "https://www.nowhere.org/index.asp";

this doesn't solve your problem. However, it may at least, show you
that some neophyte like myself was able to make it work.
