The log you've included is generated when you check the "Process Accounting" field. These "process log" transactions are logged into the same logfile as the HTTP transactions, so you'll see the #Header fields for Process Accounting and for HTTP transactions occurring repeatedly in sucj a logfile. As such, there's nothing wrong from an IIS point of view with the log you've posted, assuming there weren't any HTTP transactions to log. If you think that there were HTTP transactions, but they're not showing up in the log, then there may be an IIS problem, but I've no idea what it might be - I'd suggest turing off Process Accounting and trying again.
