My guesses, in reverse order. 1) It's probably a permissions problem. The web server user probably can't read the logfiles. 2) Errors should be written to your web server's error log. 3) "Didn't return a complete set of headers" normally means you haven't followed the instructions at the top of anlgform.pl.
