There's nothing wrong with your LOGFORMAT.

matches

... "GET /empty HTTP/1.0" 200 1101 "http://x.x.x.uk:8080/~exchange"; ...

The Request is "/empty". You don't have a FILEINCLUDE statement for /empty, so it's implicity excluded.
