Which OS and version of Eclipse?

When you plugged in the device to the computer (if Windows), you didn't
mount the device from notifications pane on the device did you?

The best setup I have found so far is Windows XP/Vista/7 with Eclipse 3.4.2
installed with the Android Plugin installed.  All you need to do with that
setup, is plug in the phone and run the application in eclipse.  I've never
had an issue but do know of several individuals who have...especially in
their *nix environments.
