I solved the problem, but I'm not sure why it was a problem.  In case
anyone is interested:

A menu selection calls a dialog box which inserts the new row.
It worked only the first time it was selected after the program
starts. None of the variables change after the first time.
I moved the identical insert code from the dialog into the menu
selection code and it works every time. The user doesn't have a
confirm dialog, but I'm not sure why I thought it was needed in the
first place.

Any idea why this was happening?
