Those numbers are unique downloads to a device, so if someone does as you suggest, a download, uninstall and redownload, that would count as one unique download. Androlib is estimating downloads based on a limited amount of data, probably by using the time it took to go to each level. Androlib has no special knowledge of the market, they get their data by scraping the market app from their server, so they see what any user would see when they shop for an app.
