Many people aren't aware they can leave a comment and many more have no idea how to leave a comment and rate an app. I've been surprised that people have written to me asking how they can do so. It seems fairly obvious to me, but then again, I have an inquisitive mind and can figure stuff out. There is a sizable portion of the population who don't go pushing buttons randomly.
What I've done in the past to prompt users to rate the app is ask them to do so in the help text and also have a link to the app's market page. In my experience, less than 1% of users will leave rate my app, a lot less than that will leave a comment (20,000 ratings, 3,000,000 downloads). I believe trying to get "everyone" to rate your app is pretty much an impossibility. Another poster said to make your app "awesome" - I think a better way to get users to comment is to make it absolutely crappy. There's a rule of thumb about customers.
