But first, try to fix it yourself by using either Toast or Log.i (these can be tough to spot in Logcat---I usually repeat the same Log "command" about 5 times, which makes it easy for even my eyes to spot) and find out what directory it's trying to save to. You may find that the directory in the path is bad. For example, depending on how you build the path to save to, you might have something like this: /mnt/sdcard/mnt/sdcard/ .... and so on. This will fail silently, leaving you wondering what happened. How do I know? I ran into this once. The Toast text I added immediately revealed the problem.
