There is a transparent proxy on T-Mobile's port 80, which works at the IP level (so it doesn't need to be explicitly configured at the HTTP level, but also can't be bypassed).
Does your apk download over wifi? Can you try serving it from a port other than 80? There's an open issue http://code.google.com/p/android/issues/detail?id=1085 about situations where small apks (less than 64kB) sometimes cause problems, though we've never been able to quite get to the bottom of it.
