It depends on what you are modifying. If you are just changing a bitmap, you can just modify that file. However, whatever change you make, you will always need to re-sign the .apk (with jarsigner) with your own cert. You probably will need to strip out the jar manifest files before doing so, so that the existing signing is not bad (mismatching the contents).
