I think you misunderstand GPS. GPS is not provided by the telephone carrier (although some location services are, such as cell tower triangulation), but from satellites. Additionally, the signal is one way from the satellite down to Earth. Thus, it is effectively free because you just have to listen for the signals and do a little math.
As for location services, I don't know of any US carriers that are charging for this separately. I assume the costs are absorbed as part of the phone hardware (and that contracts that pay for that).
