Your operator will typically handle updates for regular users. Some operators will provide automatic over the air updates, some will provide update packages.
If your phone provides root access (some phones come with it, like the Magic/ION and Dream/ADP1), or if you are able to obtain root using a hack (at the moment, I think that ALL 'droid phones are hackable for root), then yes, you can install your own or other custom builds, but you're on your own as far as support goes.
