As to the id, actually, I have -never- been required (by BoA or anyone else)
to send photo ID for an online transaction. If the addresses don't match, it
is declined until either the ship-to address is changed or it is added as an
approved address on the card. This isn't a point of sale transaction, the
rules are different..
