Those are different modes within the system. You are usually in 
MODE_NORMAL. When someone calls you, it enters MODE_RINGTONE. When you 
answer a call or you initiated a call then you enter MODE_IN_CALL. I 
should think it rather obvious from the names as to what they do.

Typically in MODE_NORMAL, you are routining audio to speakers or headset.
MODE_RINGTONE you could be sending audio out to speakers and headset. 
Flashing the screen and/or vibrating may occur as well.
MODE_IN_CALL you are routine the voice path from the modem to 
speakers/headset and mic to modem.
