How did you get into fastboot? I usually use adb rather than pull the
battery and jump through the button hoops.

When in fastboot, you need a different set of drivers. Actually, the
drivers are there but Windows does not select them. When the device is
first recognized, you have to choose the option to "Let me pick the
driver" -> "Have Disk" -> "Choose Manufacturer" and then point to the
correct driver. (Unfortunately, I don't recall which driver that was
for the HTC - try the USB driver).
