It seems the router does not attach template to <ng-view> if that Element 
is within _included_ template.
However, it does get recognized, once another <ng-view> is present in 
index.html.

Here is an example, where both <ng-view> get replaced:

And here the top <ng-view> is uncommented, which somehow disables
also the other <ng-view> inside the included template:
