I'm sorry to bother you, but my requirements have changed and now I'm 
unsure again.  Here's the new plunkr:

1.  Now my row heading and column headings are passed as two separate 
arrays from the server.  The row heading is an array of objects.
2.  The server will also return a third *array of checked items*.  Only 
data pairs within this array will render checkboxes that are checked. 
 However, all other checkboxes should still be rendered, but just unchecked.

I tried initially by adding a column index to the checked items by finding 
the indexOf the table headers and adding that to the objects within the 
array of checked items (and then maybe matching the ng-repeat $index with 
this column index).  But I don't know what to do next.

I'm really tempted to switch to full-blown jQuery for this as it's getting 
a bit complicated for my Angular brain to handle.  I'm fine in all other 
aspects of Angular, but this is really giving me a headache.
