Raniel, você precisa evitar usar `$scope.exame` como vetor e ao mesmo tempo como a variável de escopo para o exameSelecionado. Trate o vetor como `$scope.listaExame`. Consequentemente, em ng-repeat você vai usar `ex in listaExame`. Isso já vai evitar de fazer duas atribuições diferentes para uma mesma variável.
