I have reposted the document primarily to update an author's email that has
changed, and who is not getting last call review comments.
There are three minor edits relating to review comments that you can see in
the diff. In addition, the examples are being updated, and I see that the
automated conversion has not entirely done the job that I desired, with some
excess BEGIN CMS lines, and some lines not wrapped at all.
