Does it work if you use pure YAML syntax?

- name: add logon banner
  lineinfile:
    dest:
    /usr/lib/python2.7/dist-packages/horizon/templates/auth/_login.html
    regexp: '^  </fieldset>'
    line: '  </fieldset>{% include "banner.html" %}'
