Sounds like a bug in either AW or the version of the ANTLR tool you're using. 
All but one of my grammars works like your original one without any problems. 
The last uses a hash table for keywords because it's in a speed-critical area.

What's interesting - I checked the original Java grammar downloaded
from ANTLR's web page, ANTLRWorks reports errors about tokens never
matched as well. Huh?
