(parallel) pipeline requests. How does this work, how do you configure

Oops spoke too soon. I'll look at this. Is there any writeup on the new
features of 4.0, I've been reading through the code, but don't have a
complete idea of what all was done and why. For instance, I'm using the
proxy request features to handle errors, but I don't see where that gets
called during the request handling.  This thread pool based on url
sounds wonnerful.
