Lars... it doesn't seem to work if there is an error in the .htacess file.
Put in a directive that doesn't exist or a mistake and the custom error
message isn't displayed.  I have in the parent directory a Custom Error
message directive in the .htaccess file.  Then in a subdir of that
directory I purposely have an error in the htaccess file (to test it), and
I get the default message.  Can you check this specific problem?  It
doesn't work on my local Apache.
