I am not sure if she is right or not.  *Should* it find
the .en case if all she has is en-gb and ja in her
Accept-Language?

wasn't sure where to send this as I don't really want to join the full
documentation project but it's a suggestion re. improvement... so I hope
you're able to send this in the right direction.

I happen to have my browser (IE5) language preferences set to first en-gb
then second ja (odd I know, but I've been testing language preferences on
our own servers here). As a result of a web search I arrive directly at:
http://httpd.apache.org/docs/mod/directive-dict.html
and it comes up in Japanese. According to my understanding of expression
of language preferences if en-gb is not available then it should first try
en before going to my second preference of ja.
