Ok im a little familiar with this phenomenon. I notice it mostly happens 
during the development stage or when js errors occurs prior to the BAD_CHL. 
The reason been that the challenge number is increased by the framework on 
every outgoing message but if your app breaks right before the message is 
actually sent the server never gets the message. So the next message sent 
by the client may actually carry the wrong challenge number.

This is one of the reason why A LOT of people stay away from sessions as 
they are right now, they are evil.
