I found a 1995 newsgroup thread on the same card:

Some seemingly knowledgeable fellow name Tim said

It's a keyboard encoder from a Apple ][+ or very late II.  (It is
distinguished from the usual II encoder by the presence of the
CTRL-Reset switch.)


Sometimes you get lucky finding info via Google.com by just searching on the Apple part number
